Health Care Readmission

Health Care readmission students who fail or withdraw from a Health Care curriculum class must complete and submit this application in order to be considered for readmission. 

Students are only permitted two attempts in the Health Care program. A grade of W, WF, D or F is considered a program attempt. 

In order to promote academic success, students are encouraged to apply to re-enter the program for the next available term. Readmission is subject to course and space availability. Readmission applications are ONLY accepted the last two weeks of each semester.
